Important architectural decisions can be lost, forgotten, or communicated poorly. When that happens, people may make the same decision again, sometimes in a different way, and new team members may not understand why it was made in the first place.

- Capture important decisions in the Architectural Decisions work product.
- Put a process in place for making and ratifying decisions, possibly through a Design Authority responsibility.
